Excellent post. It would be wonderful if everybody could leave a good impression with proper tipping.

Make sure you budget for this everybody!!!

Oh, and if it could be named tipping for congoers, it would make a WORLD more sense right off the bat.

Additional admittedly biased opinion, from a chef:

The cook may have accidentally burned your hash browns and needs to make another batch. There may be a line at the grill, which only has so much square footage upon which to cook.

Furthermore, to note: Cooks generally do not see a thin dime of tip money. They are paid better than the waitstaff to compensate for this, of course. But if your steak was cooked just perfectly, if the bernaise sauce was stunning, if the risotto has you begging for the recipe- you might pass the waiter a $5 and say, "This is for the cook who made my meal, please- and let him/her know it was fantastic!" You will have utterly made that cook's day, I guarantee. Even a verbal "Please tell the cook this dish was fabulous." getting back to the kitchen will make them smile.

If you're going to tip the kitchen, you'd better be tipping your waitperson a solid amount as well. They're much more dependent upon that tip for their wages, as expressed. My personal rule of thumb is not to tip the kitchen unless the waitperson is being tipped 20%.
